Did you know as a patient of Windom Area Health, you have access to request a copy of your medical records? Whether you need a copy of your records for your personal use or for continuing care purposes, we have a few options available for you to obtain records:
- You can view portions of your medical record securely online through My Chart. My Chart enables you to manage and receive information about your health. A majority of test results and provider notes are set to auto release to My Chart, but there are some results and provider notes the provider has to manually release for you to view. Through My Chart, you can request a copy of those results and provider notes to be released via My Chart.

- Our Release of Information (ROI) personnel are available M-F from 7:30am- 4:30pm and can be reached at 507-831-0604 to answer any questions and take requests for records via phone. By calling ahead of time, we will prepare your records so they are ready when you arrive or we can mail a copy to you.
- When possible, we can process ROI requests for walk-ins if you are onsite and need a copy of your records. However, we encourage you to call ahead of time so we have ample time to prepare your request. If the request is from a test or visit from that same day, we will need to ensure the results are final before we can release the records. If they are not, we can get the appropriate paperwork completed that day, and mail the records when the final results are available.
In order to receive a copy of your medical records, you will be required to fill out an Authorization for Disclosure of Personal Health Information which allows us to release your records. The Authorization for Disclosure of Personal Health Information can be located on our website: https://windomareahealth.org/patients/. This form can be completed and faxed to us at 507-831-5749 or mailed to:
