Benefit Summary
Paid Time Off (PTO) Time off with pay is accrued for personal (vacation, holiday, illness, or other) needs. PTO accumulates from the date of hire and may be used only after the Orientation period is complete. A limit of 320 hours can be carried at any time. Once an employee reaches that level, they will stop accruing PTO until time is used. For purposes of the table below, one year of service is the equivalent of 2,080 hours paid.

Sick Leave Bank
Accrued bank of hours to be used for illness related to you or an eligible family member, lasting 2 or more days. Sick bank hours accumulate based on hours paid and can be carried over with a maximum accrual of 960 hours. The accrual rate is .0192 per hour paid. Sick Leave Bank hours may not be used until the Orientation period is complete.
Bereavement Leave
Full-time and regular part-time employees are eligible to receive bereavement leave. Days provided differ based on family relationship.
Holiday Pay
Time and one-half (1 ½) an employee’s regular rate of pay will be paid to any employee working on a designated holiday observance including New Year’s Day (1/1), Good Friday, Easter Sunday, Memorial Day, Independence Day (7/4), Labor Day, Thanksgiving Day and Christmas Day (12/25).
Health Insurance
Windom Area Health provides health insurance through Medica. Coverage options include a co-pay plan and a high deductible health plan option. Each plan has single, single + spouse, single + child(ren) and family coverage. WAH pays 75% – 80% of the total premium for full-time employees and regular part-time employees. Full-time employees are eligible for health insurance on the first day of the month following 30 days of employment. Part-time employees are eligible on the first day of the month following 60 days of employment. Open enrollment occurs once each year (normally in November).
Family Medical Leave
Dental Insurance
Windom Area Health provides 100%Â of the cost of dental insurance for full-time employees. Â Employees are able to 'buy up' for family members or for richer dental plan coverage. FT employees are eligible on the first of the month after 30 days.
Vision Insurance
Windom Area Health provides vision insurance through EyeMed. Coverage options include employee, employee + 1, employee + children and family coverage. Full time employees are eligible for vision insurance on the first day of the month following 30 days of employment. Part time employees are eligible on the first day of the month following 60 days of employment.
Life Insurance
Life insurance options are provided through Hartford. Employees have a guaranteed issue of $100,000 and can obtain up to $300,000 of coverage based on medical underwriting. Spouse and dependent coverage is also available. Costs vary depending on age and amount elected.
Supplemental Insurance
AFLACÂ provides a variety of insurance options. AFLAC insurance options include, but are not limited to, short-term disability, cancer protection, accident insurance, life insurance, and dental insurance.
FLEX Spending
The FSA is a pre-tax savings plan for unreimbursed medical or dependent (child or elderly) care expenses. WAH matches employee contributions into the medical plan up to $10/month for full time and $5/month for regular part-time employees.
PERA
Public Employee Retirement Association (PERA) is a mandatory retirement savings plan required because of WAH’s municipal status. Both WAH and the employee contribute to this retirement savings plan. As of 1/1/12, employees contribute 6.5% of their gross earnings and WAH contributes 7.5% of the employee’s gross earnings. Retirement benefits are based on years of service and your age at retirement.
MN State Deferred Compensation
MN State Deferred Compensation is a voluntary savings plan intended for long-term investing for retirement. This is comparable to a 401K but run through the state. Employees may contribute up to 50% of annual taxable income or $22,500 whichever is less. Employees over age 50 can contribute up to $30,000. Employees choose investment options, and pre- and after-tax (ROTH) options are available.
Employee Assistance Program (EAP)
The EAP is a confidential counseling service available to all employees and their family members. EAP services are contracted through VitalWorkLife Solutions. Contact 1-800-383-1908 for up to three (per incident, per family) free and confidential appointments.
Financing Clinic & Hospital Bills
WAH employees may designate funds to pay hospital/clinic bills through after-tax payroll deductions.
Wellness Center
All employees have free 24/7 access to the Wellness Center. Employees must purchase a key fob ($10) if they do not currently have one, and will need to use it each time they workout. Employees will get 20% off memberships for their spouse, or their household. Employees may have their spouse and household memberships paid through payroll deduction.
Physical Therapy Consultation
WAH employees may seek a complimentary evaluation from the Physical Therapy staff. Subsequent treatments for a medical concern must be ordered by a physician and will be charged as an outpatient procedure.
Personal Development Incentive Program
Windom Area Health encourages employees to seek out personal development opportunities. If an employee reads or listens to a book focusing on bettering themselves either personally or professionally, they can write a 3-paragraph essay to receive an extra $50 a book, per month.
Job Shadowing
Windom Area Health supports the development of local students interested in, or entering the healthcare industry. When possible, we will provide local students observation and job shadowing experiences upon request.
